SESSION STARTING SATURDAY, OCTOBER 5th

from 9:30am - 10:15am

MIXED SESSION - includes Ages 0-3 (Stages 1 & 2)

Stage 1 (Age 0-1): This infant/parent class will use sensory stimulation such as bounces, tapping and clapping, wiggles and tickles, accompanied by simple songs, rhymes, and lullabies, to activate different areas of the developing brain of babies who are in receptive learning stage.

Stage 2 (Age 1-3): This parent/toddler class will combined sensory stimulation's with an emphasis on more advanced body awareness and movement such as finger plays, circle time and imitation, with the use of simple songs, rhymes, and rhythms of a more classical nature to nurture babies in their development of social interactions, speech and language, and gross and fine motor skills.

These are in 6 week sessions that progress - $65/6 week session.

OPMI Parent Statement of Understanding*

BY CHECKING THIS BOX, I understand a single class is $15, 6 consecutive weeks are $65 and they can start classes at anytime, OR 10 classes for $125 which must be used anytime between now and the end of July.

OPMI Music First (parent/Payment Responsibility*

BY CHECKING THIS BOX, I understand that Parents are required to participate in ALL Infant and Toddler Classes. I also understand that class is 45 minutes in length and that session fees are due at the first class and are NON-REFUNDABLE. I am responsible for payment in full. I understand that class is once per week. I acknowledge that there will not be a pro-rated rate or refund for any portion of the session that my child and I do not attend.

OPMI Music First Liability Waiver*

BY CHECKING THIS BOX, I AGREE THAT I HAVE READ AND VOLUNTARILY SIGN THIS AGREEMENT ELECTRONICALLY and further agree that no oral representation, statements, or inducement apart from the following written agreement has been made: IN CONSIDERATION, of being permitted to utilize the facilities, services and programs of ORLANDO PREMIER MUSIC INSTRUCTION, hereinafter referred to as “OPMI”, for any purpose, including, but not limited to observation or use of facilities or equipment, or participation in any off-site program affiliated with OPMI, the undersigned, for himself or herself and any personal representatives, heirs, children and next of kin, hereby acknowledges, agrees and represents that he or she releases OPMI and its staff members from all liability for any injury, loss or damage connected in any way whatsoever to participation in OPMI activities whether on or off of the premises affiliated with OPMI. He or she understands that this release includes any claims based on negligence
